Adolescents

We begin caring for you and your family at the age of adolescence. We are proud to be a family oriented practice that aids in ushering our adolescent patients through their many phases of growth toward adulthood. We strive to provide a consistent and comfortable environment for these younger patients, so that they develop a trust and practice of medical care at an early age and stage of their development.

Adults

Our adult patients receive customized care for a variety of needs and conditions, including acute issues, chronic illnesses, and general maintenance and prevention. Our family and education forward practice sees our patients as partners to participate in the care they receive, and the decisions they make to receive that care.

Seniors

As our patients get older in their adult years, consistency and continuity in their care is perhaps the largest factor. We strive for our senior patients to have that important ongoing connection with their valued providers. Our focus on educating our patients extends to authorize the family members, to educate and involve all persons necessary to make ongoing health decisions.

REVIVE Center for Health and Wellness is focused on providing care to our patient family new and old from adolescence throughout their lives. We strive to offer services that respond to and consider each individual patient and their particular needs. We are committed to educating our patient family on our care offerings and their present health conditions and considerations. We are firm believers that wellness, care, and peace of mind all begin when we can find and relate to a care provider who is thoughtful and patient-centric.


Preventive Services

In examining the health and wellness of all patients, it can be said that prevention is sound medicine, and we work to demonstrate that in our practice. Most often, quick discovery can lead to a quicker recovery. We will utilize many methods and options for preventative care that consider the whole self.

Pegular/annual check-ups
Ongoing counseling
Screenings to prevent illness & disease
Injury assessment

Periodic and Annual Health Assessment
The team at REVIVE Center for Health and Wellness believes that our patient family should schedule and complete annual and periodic health assessments. This is a core part of our practice as a full health care assessment/checkup can examine, reveal, diagnose, and begin treatment of a wide variety of health concerns in their early stages, thereby allowing us to tailor a program of treatment and care for the patient.
Medication Management
As patients get older, or begin treatment of healthcare concerns, either on a temporary or chronic basis, the potential arises for there to be compound in conflict with prescribed medications. At REVIVE Center for Health & Wellness, we aim to educate each patient about which of their medication’s correspondence directly with which of their conditions and issues under treatment. We extend this education to members of our patient’s family and other related caregivers, creating an informed support system.


Acute and Chronic Care

If you are ill to the extent that you need to be seen as quickly as possible, our registered and existing patients at REVIVE Center for Health and Wellness have the ability to book the soonest available appointments, either via their patient portal, or by calling the office directly.

Adult Immunizations

REVIVE Center for Health and Wellness is in full participation with our greater medical community when we encourage our patient family to stay current on all available immunizations and vaccines. These are undoubtedly part of an adult and geriatric medical plan of action. Adults will often be susceptible to diseases and illnesses due to existing health conditions, lifestyle choices, age, or even travel. We would be happy to review and assess if there are any immunizations or vaccines that should be considered for every particular patient.

Geriatric Medicine
REVIVE Center for Health and Wellness not only believes in considering the whole patient during illness or checkup, but working with patients through their lives to ensure longevity and good health. This whole health approach to our senior patients’ physical and mental wellness is an important part of our care offerings. We work diligently to prevent health issues, treat injuries, and address disabilities, as well as educate patients and their families on treatments, health forecasts, and options both now and later
